A translation could be:

If you know the world
Then you know me
If you're getting lost
Then hold me tight

Well, my english is rather poor, but I think that the translation is something like this:
If you know the world,
then you know me.
If you are loosing yourself,
then stay tide by me.

I think the first one! But is it really necessary to translate the italian verse? The author has used the verse in italian, then you have to respect his will, haven't you?
